What is the T2D Network?
The Type 2 Diabetes Network is a collaborative group of healthcare professionals, researchers, organizations, and institutions working together to address Type 2 diabetes and overcome gaps in care and services in the community.
Led by the Institute for Health System Transformation & Sustainability, we’re cultivating a network focused on sharing, developing and applying innovative practices to improve outcomes for people living with Type 2 diabetes across British Columbia.

T2D Mental Health Project: Integrating Diabetes and Mental Health Care
Our T2D Mental Health Project addresses the unique challenges faced by individuals with coexisting mental health conditions and Type 2 diabetes or pre-diabetes. Funded by the Ministry of Health, this initiative enhances care through innovative partnerships with BounceBack and Small Steps for Big Changes, integrating lifestyle and mental health coaching in the community. Focused initially on communities like Prince George, Grand Forks, Summerland, and Oliver, the project aims to streamline referrals, harmonize coaching efforts, and align with provincial healthcare strategies to reduce emergency visits and facility-based care, ultimately improving patient outcomes and program sustainability.
T2D South Asian Populations Project: Empowering South Asians in Diabetes Care
The South Asian Type 2 Diabetes Round Tables, held in 2023 and 2024, highlighted the urgent need to address the disproportionately high risk of Type 2 diabetes among South Asian Canadians, particularly women and youth. This initiative, in partnership with the Canada Indian Network Society (CINS), provided a platform for South Asian youth, women, and physicians caring for South Asian populations to share their experiences and challenges in managing diabetes. The insights gathered offer critical guidance for tailoring diabetes care strategies to this demographic, emphasizing the cultural nuances and societal expectations that impact their health decisions. This project represents a crucial step toward improving diabetes prevention and self-management among South Asian Type 2 diabetes health in Canada.

T2D Prevention Activities: Proactive Community Engagement for Better Health
We are developing a learning health system for Type 2 diabetes that transitions from reactive care to proactive prevention by fostering a culture of continuous learning across the community care system. By bringing together engaged minds, we identify challenges and generate scalable solutions to address gaps in care and services. Our approach emphasizes knowledge sharing, behavior change, and community-based innovation to prevent Type 2 diabetes, aligning with our partners' and communities' priorities in areas such as advancing knowledge, capacity building, and improving health outcomes.
