Pregnancy & Type 2 Diabetes: New Global Guidance

The World Health Organization has released its first global guidelines for managing diabetes during pregnancy, including women with pre-existing type 2 diabetes.

The guidance highlights the importance of healthy eating, regular physical activity, and close blood sugar monitoring to reduce risks for both mother and baby. Good glucose control can lower complications during pregnancy and reduce future type 2 diabetes risk after birth.

Comparing guidelines

Australia’s updated 2024 gestational diabetes guidelines recommend higher glucose thresholds and targeted early screening to reduce overdiagnosis (see Australia guideline). In contrast, Canada’s 2020 guideline retains lower diagnostic cut-offs, offering flexible 1‑ or 2‑step screening and detailed care protocols (Canada guideline). The UK follows NICE’s NG3 guideline (2020), emphasizing preconception planning, targeted screening based on risk factors, blood glucose monitoring, dietary and lifestyle management, and multidisciplinary care NICE.

Comparisons:

  • Screening thresholds: Australia uses higher cut-offs; Canada uses lower thresholds with options; UK applies risk-based OGTT per NICE NG3 .

  • Approach: All three endorse early detection, glycemic control, multidisciplinary teams, and postnatal follow-up.

  • UK specifics: NICE NG3 adds preconception advice, ongoing fetal monitoring, and postpartum glucose testing in line with the Canadian emphasis on follow-up .
